We have transitioned to a new Electronic Medical Record and Scheduling System. 
Everyone will need to fill out new paperwork including the Medical History Form and Patient Information Form. Please bring your insurance card and photo ID with you to the appointment so they can be scanned into the new system. Thank you.

Patient Access to Records
With the recent switch to a new electronic medical record system, we are working on getting all records in our new system but may have a combination of paper records and electronic records in an old system. We request you fill out a records request if you would like a copy of your records or wish to have them sent to another provider. Please use the form below.

My Patient Visit Portal is for our older electronic records system. You can still have access to this portal to make online payments for past due account balances.  We are in the process of establishing a new portal system with the new scheduling system but it is not live yet. Please call the office if you have medical questions or need to schedule an appointment.
